Fourth year medical students can apply for reimbursement for travel expenses incurred while visiting Wisconsin Residency programs. The more programs that are visited the greater the amount of reimbursement available - up to $1,000.
Students must apply for the Wisconsin PassPort Program by completing an application form. Reimbursement is available on a first come, first serve basis. Students must submit receipts and a reimbursement request form after visiting the residency programs.

Students must complete the Wisconsin PassPort application form and either email, fax or mail the form to the Wisconsin Academy of Family Physicians (WAFP). The application form must include name, address, phone number, email address and medical school along with an indication as to which programs the student will visit. Once received by WAFP, the student will be notified as to whether or not reimbursement is available (funds will be available on a first come, first serve basis). The student will also be notified of the maximum reimbursement available. Each residency program (including rural training tracks) identified by the student will be notified of the student's participation in the PassPort Program. The residency programs will notify the WAFP of a visit by the student.
Students accepted to participate in the Wisconsin PassPort program will be asked to submit receipts of their travel expenses along with a Wisconsin PassPort reimbursement form. Once the receipts and reimbursement forms are received and the student's visit to the residency programs is verified, the WAFP will send the student a check in the amount stipulated in the schedule above.
